What XPand Optimize™ Is
XPand Optimize is testosterone replacement therapy (TRT) and broader hormone optimization for men. Low testosterone is a physiological condition. When blood tests confirm it, there is a medical way to treat it.
The aim is to restore testosterone to a healthy physiological range, the level a healthy man would have. It is not about pushing past normal or chasing a number above where the body is meant to be.
Men come to XPand Optimize for concrete reasons. Low energy. Lower sex drive. Loss of muscle. More body fat that will not shift. Low mood and poor concentration. Fewer morning erections. These can be symptoms of low testosterone, also called hypogonadism. They can also have other causes, which is why diagnosis starts with bloodwork rather than symptoms alone.
XPand Optimize is lab-driven and medically supervised. You do not start therapy until labs confirm low testosterone, and you stay on a monitoring schedule for as long as you are on treatment.
Who It Is For
XPand Optimize is for men with low testosterone confirmed on bloodwork, plus symptoms that match. Labs and symptoms together. Not symptoms alone, and not a lab number with no symptoms.
It is not for men with normal testosterone who want to push levels above normal for an edge. That is not what this program does, and it carries risk without medical benefit.
Some men are not candidates, or need to address something first. TRT may not be appropriate for men with untreated prostate cancer, untreated breast cancer, severe untreated sleep apnea, uncontrolled heart failure, or an already elevated red blood cell count (hematocrit). Men who are trying to conceive should know that TRT can reduce sperm production, and should ask about alternatives at consultation. Candidacy is confirmed through labs and an exam.
How the Program Works
XPand Optimize is built around lab data and ongoing monitoring, not a one-time visit.
Consultation
You start with a confidential consultation. Dr. Adajar reviews your symptoms, your health history, and your goals, and orders the right bloodwork.
Bloodwork and Diagnosis
Diagnosis of low testosterone is made from blood tests plus symptoms, not from how you feel alone. Initial labs typically look at testosterone and related markers so there is a clear baseline before anything starts.
Personalized Protocol
If labs and symptoms confirm low testosterone, you and Dr. Adajar set a protocol. Testosterone can be delivered by injection, topical gel, or pellets. The method is chosen at consultation based on your labs, your preferences, and what fits your routine.
Monitoring
Once you start, you stay on a monitoring schedule. Repeat bloodwork tracks testosterone along with hematocrit, PSA, and estradiol so the protocol can be adjusted and side effects caught early. Monitoring is more frequent at the start and settles into a regular cadence once your levels are stable.
Is It Safe
TRT is a regulated, medically supervised therapy. Done correctly, it is guided by labs from the first visit and monitored for as long as you are on it. That monitoring is the main safety tool.
As with any medical therapy, there are risks and side effects. These can include acne, fluid retention, an increased red blood cell count (hematocrit), reduced fertility and sperm production, testicular shrinkage, and possible effects on cholesterol. Regular bloodwork is how these are watched for and managed.
TRT is not appropriate for everyone. Men with untreated prostate cancer, untreated breast cancer, severe untreated sleep apnea, uncontrolled heart failure, or elevated hematocrit may not be candidates, or may need those conditions addressed first. These are reviewed in detail at consultation.

What to Expect and Realistic Timeline
TRT is not a quick fix, and the changes do not all arrive at once.
Some symptoms can improve within the first several weeks. Energy, sex drive, and mood often respond earlier than other changes. Body composition takes longer. Gains in muscle and reductions in body fat build over months, and depend on your training and habits as well as your labs.
Most men see the fuller effect over roughly three to six months as the protocol is adjusted against your bloodwork. This is ongoing therapy. If you stop, the benefits fade over time as your levels return to where they were. Individual results may vary.
