Scrotox in Chicago: XPand Smooth Scrotal Relaxation

  • Smoother appearance
  • Less tightness and sweating
  • Under 30 minutes
  • Topical numbing only

XPand Smooth™ (Scrotox) uses targeted injections of botulinum toxin, the same neuromodulator commonly known as Botox, to relax the muscle in the scrotum. As that muscle relaxes, the scrotum sits in a smoother, more even, more relaxed state. The treatment takes under 30 minutes in our Chicago office with topical numbing only. No surgery, no general anesthesia, no overnight stay. Dr. Marc Adajar, a board-certified surgeon (FACS) with over 20 years of experience, performs every treatment. Results are temporary, which means you see exactly what the treatment does before deciding whether to maintain it.

  • Specializes exclusively in men
  • Patients' Choice Award, 9 consecutive years
  • Topical numbing only, immediately reversible

What Does XPand Smooth (Scrotox) Actually Do?

XPand Smooth relaxes the scrotal muscle, it does not change your anatomy.

The scrotum has a thin layer of smooth muscle called the dartos. That muscle is what makes the scrotum tighten, wrinkle, and pull up in response to cold, stress, or arousal. XPand Smooth uses botulinum toxin to reduce that muscle tone. When the dartos stops contracting as hard, the scrotum rests in a smoother, more relaxed position. This is a physiological change in how the tissue rests, not a change to your underlying structure.

It does not increase testicular size.

XPand Smooth does not make the testicles bigger and does not add volume. It relaxes the surrounding muscle so the scrotum hangs more naturally, which can read as fuller and smoother. If your goal is added testicular size, this is not the right treatment, and Dr. Adajar will tell you that directly.

The treatment targets appearance and comfort together.

Most men who research this are after one of two things, and often both. The first is a smoother, less wrinkled scrotal appearance. The second is relief from the tightness, pulling, and discomfort that comes with chronic scrotal retraction. XPand Smooth addresses both because they trace back to the same muscle.

Many men also report less sweating.

Botulinum toxin affects the sweat glands in the area it treats. Patients commonly report reduced scrotal sweating after XPand Smooth, which tends to show up as improved comfort during workouts, in warmer weather, and over long days. This is a reported effect, not a guaranteed one, and individual results may vary.

Results are temporary, and that is part of the design.

Botulinum toxin wears off over time. The effect of XPand Smooth typically lasts 3 to 4 months on a first treatment. That built-in timeline means you are never committed to a permanent change. You see what the treatment does, decide whether it fits, and choose whether to maintain it on your terms.

How XPand Smooth Works

XPand Smooth works by relaxing one specific muscle. Here is the mechanism in plain terms.

The dartos muscle sits just under the skin of the scrotum. It is a smooth muscle, which means it contracts on its own without you controlling it. That is why the scrotum tightens in the cold or pulls up under stress. For some men, that muscle stays tense more often than it relaxes, which leads to a tight, wrinkled appearance and ongoing discomfort.

Botulinum toxin temporarily blocks the nerve signals that tell the dartos muscle to contract. With those signals reduced, the muscle relaxes. The scrotum then rests in a smoother, more relaxed, more even state. The effect is local to where the injections are placed and wears off gradually as the body clears the neuromodulator.

Nothing is cut, removed, or permanently altered. Because the change is muscular and temporary, the tissue returns to its baseline once the treatment wears off. That reversibility over time is a core part of why the treatment is low-commitment compared to surgical options.

Individual results may vary based on your anatomy, your baseline muscle tone, and how your body responds to the neuromodulator.

Who Is a Good Candidate for XPand Smooth?

Most men who research Scrotox are already good candidates. If you are in general good health and want a smoother scrotal appearance, relief from tightness, or both, this treatment is designed for you.

Good candidates tend to share a few things:

  • A specific appearance or comfort goal. You want a smoother, more relaxed scrotal appearance, or you are bothered by wrinkling, tightness, or retraction that does not let up.

  • Daily discomfort from scrotal retraction. You notice pulling or tightness during exercise, sitting, or intimacy and want relief.

  • Bothered by scrotal sweating. You want to reduce sweating that affects your comfort during workouts or in warm weather.

  • Realistic expectations. This relaxes a muscle for a temporary period. It does not increase testicular size and does not permanently change your anatomy.

You are not a candidate if you have an active infection in the treatment area or a known sensitivity to botulinum toxin. If either applies, Dr. Adajar will recommend the right path during your consultation rather than proceed.

Some men consider XPand Smooth alongside other XPand treatments to address function and appearance together. If that applies to you, the consultation is the place to map out what fits.

If you are uncertain whether you fit the criteria, that is what the consultation is for. Dr. Adajar reviews your anatomy, health history, and goals to determine your specific candidacy. There is no obligation and no pressure to schedule the same day.

If you have spent time wondering whether wanting this is reasonable, you are not alone. Scrotal tightness, wrinkling, and sweating are physical issues with a medical solution. Treating them is a practical decision about comfort and appearance, not a vanity problem. The decision is yours, and it is a legitimate one.

A consultation determines your specific candidacy. Book a confidential consultation.

Is XPand Smooth (Scrotox) Safe?

XPand Smooth is performed in a controlled medical setting by a board-certified surgeon using botulinum toxin, a neuromodulator that has been used in medical and aesthetic treatment for decades.

Dr. Marc Adajar, MD, FACS, performs every XPand Smooth treatment at XPand MD™. He is a board-certified surgeon with over 20 years of experience who specializes exclusively in men. His practice covers the full scope of male body contouring, sexual wellness, and performance optimization, including surgical procedures, penile enhancement, and testosterone replacement therapy. He is board-certified by the American Board of Surgery, a Fellow of the American College of Surgeons (FACS), a member of the American Academy of Cosmetic Surgery, and holds additional certification in anti-aging medicine. That means a surgeon trained and tested to operate in a hospital is doing your injection, not a technician who added it to a service menu. He has also completed specialized training and certification in male genital enhancement techniques, including penile filler injection (XPand Girth™), platelet-rich plasma therapy for penile rejuvenation (XPand Restore™), and scrotal neuromodulator therapy (XPand Smooth). He has received the Patients' Choice Award for nine consecutive years.

The honest regulatory picture.

Botulinum toxin products are FDA-approved for a range of medical and aesthetic uses. Scrotal use is considered off-label, which means the drug itself is approved but this specific application is not a labeled indication. Off-label use is common and legal in medicine when supported by clinical judgment, and scrotal neuromodulator treatment is generally well tolerated with minimal reported risks. You should understand that distinction before you decide, and Dr. Adajar covers it directly in your consultation.

The treatment uses topical numbing only. There is no general anesthesia, which removes the risks that come with sedation. Treatment happens in a private office setting at our River North location, not an operating room, and typically takes under 30 minutes.

Honest risk disclosure. Common temporary side effects include mild swelling, bruising, and tenderness at the injection sites, which usually resolve within a few days. As with any botulinum toxin treatment, there is a rare possibility of the effect spreading beyond the intended muscle or producing an uneven result. Serious complications are rare when the treatment is performed by a qualified, experienced provider in a medical setting. No procedure is risk-free, and the right provider is the single biggest factor in a safe, even result.

Provider expertise matters. Scrotal injection requires accurate placement of the neuromodulator in the right muscle layer. In untrained hands, poor technique can lead to an uneven result or unintended effects. The difference between a clean result and a complication is almost always the provider. Patients should seek a board-certified surgeon who understands male anatomy and performs these treatments regularly, not a general injector who added it to a menu.

A confidential consultation with Dr. Adajar covers your specific anatomy, goals, and any medical history that may affect candidacy. Every question gets a direct answer before you commit to anything. Individual results may vary.

What Happens During the Procedure?

The entire visit takes under 30 minutes in the office. There is no general anesthesia, no operating room, and no overnight stay. Here is what to expect, step by step.

  1. Step 1: Consultation

Dr. Adajar reviews your goals, anatomy, and health history, then confirms whether XPand Smooth alone or a combination of treatments is the right call. This is where you confirm the plan and ask any remaining questions.

  2. Step 2: Topical numbing

A topical local anesthetic is applied to the treatment area so the scrotum is numbed before any injection. You stay awake and alert the entire time.

  3. Step 3: Injection

Dr. Adajar places targeted injections of botulinum toxin into the dartos muscle. The treatment is performed in-office and typically takes under 30 minutes from start to finish.

  4. Step 4: You leave

There are no stitches, no drains, and no bandages to manage. You return to normal activity the same day. Avoid hard workouts and sexual activity for 24 hours, and avoid direct pressure or manipulation of the area right afterward.

How much it hurts

Pain is the most common concern men raise before booking. The scrotum is numbed with topical local anesthesia first, and most patients report minimal discomfort during the injections. The treatment is well tolerated.

Recovery

You can return to desk work and light daily activity the same day. Hold off on hard workouts and sexual activity for 24 hours to let the area settle. There is no bed rest, no surgical drain, and no general anesthesia recovery to manage. Dr. Adajar provides specific guidelines at your appointment based on your treatment.

The treatment is performed at the River North office in Chicago. You arrive, numb, treat, and leave, usually within the hour. No one in the waiting room knows why you are there.

What to Expect: Onset and How Long Results Last

Results do not appear instantly. Botulinum toxin takes a few days to take effect as it relaxes the muscle.

Onset. Most men begin to notice changes within 3 to 7 days. The full effect usually develops over 1 to 2 weeks as the dartos muscle relaxes completely. This is normal and expected, so plan around that window rather than judging the result on day one.

Duration. A first treatment typically lasts 3 to 4 months. As the neuromodulator gradually wears off, the muscle returns to its baseline tone and the scrotum returns to how it rested before treatment. The change back is gradual, not sudden.

With repeat treatments, results can last longer. Many patients who continue treatment see effects lasting 5 to 6 months over time. The muscle responds more consistently with repeated, spaced treatments.

Individual results may vary based on your anatomy, baseline muscle tone, metabolism, and how your body responds to the neuromodulator. Dr. Adajar discusses expected onset and duration based on your specific plan during your consultation.

How Much Does XPand Smooth (Scrotox) Cost in Chicago?

$1,800a la carte

The price is lower when XPand Smooth is combined with other XPand treatments in a single plan.

We publish this number because you should have it before you walk in the door. Many clinics hide pricing behind a consultation wall, which makes comparison harder and wastes your time.

What the cost covers. Your fee includes the botulinum toxin, topical numbing, the injection treatment performed by Dr. Adajar, and your follow-up. You are awake the entire time in a private treatment room. There is no separate facility fee for the treatment itself and no general anesthesia charge, because the treatment does not use general anesthesia.

Combining treatments lowers the per-treatment cost. XPand Smooth is often combined with other XPand services. The XPand Performance Collection bundles XPand Flow (penile neuromodulator for blood flow and function), XPand Smooth (scrotal neuromodulator), and XPand Restore (platelet-rich plasma penile rejuvenation) at package pricing below the a la carte total. If girth is also a goal, XPand Girth (penile filler) can be added.

Because results are temporary, retreatment is part of the plan if you choose to maintain them. The first treatment is the real decision. After that, you already know what the result looks and feels like, so a maintenance visit is a smaller, known commitment rather than a fresh gamble.

See our full pricing breakdown for all XPand services.

Longevity and Maintenance

XPand Smooth is a temporary treatment by design. Botulinum toxin relaxes the muscle for a window of time, then gradually wears off.

A first treatment typically holds for 3 to 4 months. Over that period, the muscle slowly regains its normal tone and the scrotum returns to how it rested before treatment. There is no sudden change, just a gradual return to baseline.

Maintenance treatments extend and build on the effect. Many men schedule a repeat treatment before the effect fully wears off to keep the result consistent. With repeated, spaced treatments, the muscle tends to respond more predictably, and many patients see results lasting 5 to 6 months over time.

Maintenance is optional. If you try XPand Smooth once and decide it is not for you, you simply stop, and the effect wears off on its own with no reversal procedure required. That low-commitment structure is part of what makes the treatment lower-risk than permanent options.

Individual results may vary. Dr. Adajar discusses an expected maintenance cadence based on your specific response during your consultation.

Why Choose Dr. Adajar and XPand MD?

Dr. Marc Adajar, MD, FACS, is a board-certified surgeon who specializes exclusively in men. Every surgery he performs, every treatment he administers, and every optimization protocol he designs is for male patients. His practice spans male body contouring, sexual wellness, and total-body performance optimization through testosterone replacement therapy. He is board-certified by the American Board of Surgery, a Fellow of the American College of Surgeons (FACS), a member of the American Academy of Cosmetic Surgery, and holds additional certifications in anti-aging medicine and male genital enhancement procedures, including penile filler injection (XPand Girth), platelet-rich plasma penile rejuvenation (XPand Restore), and scrotal neuromodulator therapy (XPand Smooth). He has been in practice for over 20 years and has received the Patients' Choice Award nine consecutive years.

Those credentials matter because scrotal injection requires accurate placement in the correct muscle layer. A surgeon who specializes in male anatomy and performs these treatments regularly can place the neuromodulator so the result is even and the effect is targeted.

One surgeon, not a rotation. Dr. Adajar performs every XPand Smooth treatment himself. You will not be handed off to a physician assistant or nurse injector. The same hands that evaluate your anatomy in the consultation are the ones doing your treatment.

Built exclusively for men. XPand MD is not a general aesthetics clinic that added scrotal Botox to a menu of facials and lip injections. Dr. Adajar treats only male patients. The entire XPand system is designed around male physiology:

  • ·XPand Girth: penile filler for girth enhancement
  • ·XPand Flow: penile neuromodulator for blood flow and function
  • ·XPand Restore: platelet-rich plasma therapy for sensitivity and tissue health
  • ·XPand Smooth: scrotal neuromodulator (Scrotox) for comfort and a smoother appearance
  • ·XPand Optimize™: testosterone replacement therapy

Having these services under one surgeon means your plan accounts for the full picture, not just one piece of it. If you raise other goals during your consultation, Dr. Adajar can address them in the same plan instead of sending you to a separate provider.

Discretion is built into the process. The clinic is on the second floor of a River North medical building. There is no signage that identifies the treatment. Check-in, treatment, and checkout happen in a private suite. Dr. Adajar performs the treatment himself. Most patients are in and out within the hour.

The XPand Protocol: One Part of a Modular System

You came here for XPand Smooth. It addresses scrotal comfort and a smoother appearance. But Dr. Adajar treats the full picture, and most men who look into one XPand treatment find there is more available to them than they realized.

The XPand Protocol™ is not a sequential program. It is a modular set of treatments, each targeting a different aspect of male function, performance, and physical results. You choose what fits your goals. Some men do one treatment. Others combine two or three.

Dr. Adajar performs all of them, so your plan does not get split across multiple providers.

If you want to address function and aesthetics together, XPand Flow, XPand Restore, and XPand Smooth are available as the XPand Performance Collection, a bundled package priced below the a la carte total.

Your consultation is the right time to ask about any of these. Dr. Adajar reviews your goals and health picture and tells you directly which treatments, if any, make sense to combine with XPand Smooth. Book a confidential consultation.

Frequently Asked Questions

Scrotox is the common term for scrotal Botox, a treatment that injects botulinum toxin into the muscle of the scrotum to relax it. As the muscle relaxes, the scrotum sits in a smoother, more even, more relaxed state. XPand Smooth is XSculpt’s branded protocol for this treatment, performed by a board-certified surgeon.

It relaxes the dartos muscle in the scrotum using botulinum toxin. The dartos is the smooth muscle that makes the scrotum tighten and wrinkle in response to cold, stress, or arousal. When the neuromodulator reduces that muscle’s contraction, the scrotum rests in a smoother, more relaxed position. Nothing is cut or permanently changed, and individual results may vary.
